Additional Services
The BVCHC Community Health Team works with clinical care teams to provide whole person centered care to patients. Community Workers and Behavioral Health Specialists assess patients for barriers to achieving good health and work to address these barriers as part of patient care plans. The Community Health Team helps patients access community resources and navigate health care, promotes health behavior change, and provides social support to our most vulnerable patient's in the community.

After Hours Care
BVCHC providers are available daily during and after hours at 401-722-0081. A provider is available after hours for important medical issues.
Express Health Care
Express Health is available by appointment only in Central Falls. Express Health Care is available Monday through Friday, 8am to 5pm, and walk-in only on Saturdays, 8am to 4:30pm. For Monday-Friday appointment, please call first, 401-722-0081.
