The Behavioral Health Supervisor provides clinical supervision to behavioral health licensed staff and interns in addition to direct clinical service to patients in an integrated team based model of care. Clinical service may include consultation/assessment(urgent basis or pre-scheduled), support and linkage services,psychoeducation,.brief interventioins, and conuseling to enhance patient physical and psychological well-being. The Behavioral Health Supervisor participates n quality improvement and peer reviews, and assists in developing programming as required by the Director fo Behavioral Health Services..
The Community Health Worker – Family Support Specialist is a member of the Community Health Team and acts as link to community-based organizations to facilitate patient access to health/social services.

The Quality & Population Health Analyst is responsible for supporting practice transformation through process improvement and health information evaluation. This position will work within the Health Information Management (HIM) department to identify, analyze, and execute initiatives to ensure best-practice processes and optimal HIT use throughout the clinic. This position will conduct business or project needs assessments and interpret initiative results to drive performance improvement through a variety of health information tools. The Quality and Population Health Analyst engenders a culture of continuous quality improvement, innovation, and practice change that enhances a patients’ clinical outcomes as well as patient and staff experience.
