Nurse Case Manager
Description: Full-Time. Responsible for case coordination and chronic disease management/education. Works with interdisciplinary teams. Reports to the Director of Clinical Operations.
Qualifications: Current Rhode Island RN license.
Requirements: Three years nursing clinical experience. Two years case management experience. Excellent communication skills. Computer skills. Bilingual skills a plus.
Registered Nurses
Description: Full & Part-time. Responsible for providing nursing clinical services at one of our locations. Reports to the Clinical Nurse Manager.
Qualifications: Current Rhode Island RN license.
Requirements: Nursing clinical experience. Excellent communication skills. Computer skills. Bilingual skills a plus.
Physicians (Family Practice, Internal Medicine)
Description: Full & Part-time. Responsible for physician clinical services at one of our locations. Excellent salary & benefits, educational loan repayment. Reports to the Medical Director.
Qualifications: Holds or eligible for Rhode Island Medical license.
Requirements: Experience in ambulatory setting. Bilingual skills a plus.
